Good Shepherd Hospice admits patients who have been diagnosed with life-limiting illnesses and have a prognosis of one year or less. Though most hospice insurance benefits only cover services for the last six months of life, if individuals meet Good Shepherd Hospice's admission criteria, they will receive care regardless of their ability to pay. Admission Criteria

• Patient is diagnosed with a life-limiting illness
• Patient's prognosis is one year or less - if the disease takes its natural course - as confirmed by an attending physician
• Patient is primarily interested in a palliative plan of care
• Patient and family desire Good Shepherd Hospice services

Referral Information

Usually patients are referred to Good Shepherd Hospice by their primary care physician. However, the process can be initiated by other health care /professionals, family members, friends, clergy or by the patients themselves.
